Go Back INFORMATION FOR INDIVIDUALS WITH ASTHMA : Participants in this project will receive:
	An initial home visit by a community health worker. During this visit, the health worker will interview you and assess your home for possible asthma triggers.
	An asthma kit with products to help you control and monitor your asthma symptoms.
	Individualized instruction designed to minimize or eliminate asthma triggers from your home.
	A follow-up visit six months after the initial visit to find out how much relief has been experienced from the asthma symptoms.
This program is designed to help a limited number of individuals in the target communities on a first-come, first-served basis.
